For a quick key explaining the different countries listed see below:
- Country on top of documents = Driver's License Country of Issue (Home country license)
- 3rd Line on Headshot Page = Place of Birth
- 5th Line on Headshot Page = Driver's License Country of Issue (Home country license)
If either your Driver's License Country of Issue (Home country license) or Place of Birth are listed incorrectly - refer to the steps below:

For example:

** Please note that for the password, the first letter of the last name must be capitalized.
